Skilled Nursing
Swan Home Health Care employs experienced Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide skilled care for our patients and educate them on topics related to their health, recovery, and disease process. Our nurses work closely with the patient’s primary physician to make sure the services they receive meet their individualized needs.

Swan Home Health Care offers therapy services designed to help patients recover function at home according to their specific needs. Each therapist conducts an assessment and sets individualized goals for a patient based on their diagnosis and goals for improvement.
Medical Social Work Services
Swan Home Health Care’s medical social work services offer additional social and emotional support for patients and their families during their illness and recovery. Our social workers also identify the appropriate community resources that a patient may benefit from during their recovery.
Certified Home Health Aides
Swan Home Health Care offers certified home health aides who help patients with activities of daily living, range of motion exercises and helping patients adhere to the home exercises prescribed by our therapists.
