New Patient Process

Becoming a Swan Home Health Medical Housecall patient starts with a conversation with one of our intake coordinators. They will do an initial prescreening to determine if you or your loved one are eligible for our services. If those criteria are met, the intake team will then take you through the necessary steps to complete the new patient process. Once you or your loved have been accepted as a patient, the patient account is put into the waiting list. Time on the wait list can vary depending on several factors. However, your intake coordinator will be able to give you an estimate of how long that wait may be. Your intake coordinator can answer any questions you may have when completing this process.

Once you have been assigned to a clinician, they will call to set up your first appointment. On the first visit, we conduct a complete medical history review interview as well as a physical examination - please allow for at least an hour for this first visit. Please be sure to have someone present who can sign forms and help give medical information if that is something you are not able to do on your own. During the initial visit, we normallly review advance directives, and discuss treatment wishes in case of medical emergency. Our goal is to have a signed Physicians Order for Scope of Treatment (POST) for every patient who wants one.

On the first visit, the assigned clinician will become the your new primary care provider - replacing any previous PCPs.
